The notice of situation or the final registration certificate in the SIRENE Directory for your establishment, showing your APE code (⚠️ APE codes 7010Z or 000Z are not accepted).
👀 Lost your document? No worries: you can download it directly from the INSEE website!

This CNTR document containing your details, to print and fill in.
- Depending on your activity:
- For businesses: An original extract from the Trade Register (K, Kbis or Lbis) for the establishment, issued within the last month. 👀 Can’t find this document? You can request it from the Registry of the Commercial Court your establishment depends on, or order it from www.infogreffe.fr (⚠️ Any KBIS extract that does not mention the activity carried out and the establishment concerned by the application will be rejected)
- For tradespeople: A copy of the extract of registration in the Trades Directory.
- For associations: A copy of the articles of association mentioning the catering activity
